PLEASE SCHEDULE YOUR APPOINTMENT CAREFULLY AS A $50 NON-REFUNDABLE DEPOSIT IS REQUIRED
Especially due to the fact that we have several locations it is crucial that scheduling be done carefully.
— A $50. non-refundable deposit is required in order to schedule your"initial" appointment.
— A $50. non-refundable deposit is required in order to schedule your "follow-up" appointment.
— Your $50. deposit will be "credited" toward the cost of your desired procedure.
TYPES OF PAYMENT(S) ACCEPTED:
— BALANCE IS DUE THE DAY OF THE PROCEDURE VIA CASH, CHECK, OR MONEY ORDER ONLY!
— NO OUT-OF-STATE CHECKS
— Credit cards accepted for Deposit Only...NOT REMAINING BALANCE.
A 72-hour cancellation notice is required to cancel an appointment. If you fail to cancel your appointment within 72 hours, please know that we will then require "full payment of the desired procedure" in order to reschedule. And, again, an additional non-refundable deposit of $50. (as part of full procedure cost) will be required to schedule a new appointment.RESCHEDULING / LATE POLICY:
If you are late to your appointment, it may be necessary to reschedule your appointment. If you choose to reschedule or postpone your appointment, a minimum 48 hour notice is required or else your deposit becomes non-refundable..
And, if you do not reschedule, you will lose your $50. deposit and future appointments will require another $50. deposit in order to secure your appointment.NO SHOW POLICY:
All "no show's" for scheduled appointments will forfeit their $50. deposit and will be required to pay "in full" the cost of their desired procedure, plus the non-refundable deposit of $50 (as part of full procedure cost) again in order to reschedule another appointment. VISITORS AND CHILDREN
The CDC and OSHA guidelines do not allow observers in the procedure room. Family and friends can remain in the waiting room, if desired. They are, however, welcome to participate in the makeup designing portion of the procedure. Wisconsin and Illinois prohibits the tattooing of persons under the age of 18. The entire office, including the waiting room, is not equipped for small children or babies. Childcare arrangements must be made prior to all appointments. FAILURE TO DISCLOSE
Failure to disclose pertinent medical or other information which results in a last minute cancellation of a scheduled procedure will also forfeit their $50 non-refundable deposit. If you have any condition in which you are under a doctor's care, you must disclose this before scheduling an appointment so that we can determine whether or not you are a candidate for your scheduled procedure.
